How can I return or exchange a product?
This policy covers courtesy returns and exchanges by customers in the United States and Canada for all products sold by Zimmer Biomet Dental (“Qualified Products”), with the exception of products derived from human tissue and patient specific products. This policy does not address returns and exchanges in connection with product non-conformances. Please contact Customer Service to complete a Product Experience Form in such circumstances.
A purchaser may return any Qualified Product within 180 days of invoice date.
A purchaser may exchange any Zimmer Biomet Implant as long as the product has a minimum six months of remaining shelf life and is otherwise in resalable condition. These products may be exchanged for other Qualified Products within the same product families as determined by Zimmer Biomet (e.g. e.g. T3® for T3®, Trabecular Metal™ or Eztetic®, etc.) provided that if the replacement products are of greater value than the returned products, the purchaser will be charged the price difference.
All other Qualified Products may be exchanged within 365 days of the invoice date for other Qualified Products within the same product families as determined by Zimmer Biomet (e.g., abutment for abutment, membrane for membrane, etc.), provided that if the replacement products are of greater value than the returned products, the purchaser will be charged the price difference.
The value of a replacement product will be determined based on the following in order of priority:
- Promotional discount applied to the purchase of the returned item so long as the original promotional code or pricing contract also covered the replacement product.
- List price at time of order.
Freight charges will apply to exchanged product shipments.

Product Experience & Complaints
How can I submit a product complaint?
All customers Outside of the United States shall complete the
applicable Product Experience Form (PER) by language (see below for
PER’s by language). One Product Experience Form (PDF) shall be
completed for each patient/event. The completed PER with the
sterilized product will be sent to the applicable complaint handling
contact (found on page three of the PER form) or your local
distributor. Once the complaint has been received by mail or
email a compliant number will be generated. If not previously
provided, the compliant product (including the complaint number) will
be sent to the appropriate complaint handling site (found on page 3 of
the PER form) or your local distributor.

Return and Exchange Polices & Warranties
Zimmer Biomet Dental Implants Lifetime Implant Warranty Program is the global limited warranty program for Zimmer Biomet Dental.
For US & Canada Based Customers
(This program is only available for US and Canada based customers.)
If you have contacted Zimmer Biomet Dental Customer Service within 90 days from the day of surgery time following attempted implant placement, a Courtesy Implant Replacement is available for a nominal fee. If the requested implant is from the T3®, Trabecular Metal™, Eztetic™ or Osseotite® 2 product families, the fee will be waived. Freight charges are complimentary for replacement implants. The customer must follow the Courtesy Implant Replacement Process when the issue is an implant dropped in surgery, one that has a loss of sterility due to opening it, but not placing, or one placed and removed immediately during surgery. A Courtesy Implant Replacement Request form must be submitted in addition to obtaining an RMA from Zimmer Biomet Dental Customer Service at 800-342-5454.*
*Product must have a minimum of 60 days of shelf life remaining as
of the date of initial contact with Zimmer Biomet Dental Customer Service.
This policy does not address the replacements in connection with:
- Product non-conformances
- Lifetime Warranty Policy.
In those cases, please complete a Product Experience Form (PER).
For US & Canada Based Customers

Effective May 9, 2017
The benefits described herein are in addition to all benefits included under the Zimmer Biomet Dental Lifetime Implant Warranty Program and are subject to modification or discountenance at any time in Zimmer Biomet’s sole discretion. The benefits herein apply only to USA and Canada customers treating patients within the USA and Canada. Zimmer Biomet reserves the right to interpret this policy in its sole and absolute discretion and may limit benefits to the extent we determine any claim for benefits is insufficient to meet eligibility requirements.
This benefit covers only dental implants manufactured by Zimmer Dental, Inc. or Biomet 3i, LLC that have been surgically placed and restored exclusively with compatible ZIMMER BIOMET restorative components. Zimmer Biomet will provide a cash reimbursement of up to $500.00 per failed T3®, Trabecular Metal™, Eztetic™ or Osseotite® 2 implant (“Qualified Products”) for actual lab fees incurred to construct the original prosthetic appliance corresponding to the failed implant. The reimbursement will be made after:
(a) receipt of a properly completed Restorative Reimbursement Application (with a copy of the associated itemized laboratory invoice),
(b) approval by Zimmer Biomet Warranty Administration.
BellaTek Bars used to restore Qualified Products are eligible for reimbursement hereunder.
